Access to participant statements is Account ID and Password protected. To establish an Account ID for your organization, please fill out this Form and mail it to:

The Form contains a field for specifying the e-mail address that will serve as your organization's Account ID. Upon receiving this Form, the TOD will use the e-mail address provided to send your organization a temporary, modifiable Password.
